4). Uninsured: Sliding
Fee Scale Program POLICY :
The sliding fee scale program
is a program that has clear guidelines
that allow Oakland Primary Health Services,
Inc. a way to reduce patient’s payment
for services, based on income and family
size. The guidelines are based on
the current poverty guidelines as established
by the federal government. Patients shall have the
opportunity to apply for the Family Assistance Program,
at the time of their appointment. The fee reduction
is identified in increments of 20%, 40%, 60%, 80%
and 100%. A minimum payment of fifteen (15) dollars
for medical services is required for all individuals
who qualify for a fee reduction. The
sliding fee scale will be updated annually and quantified
with the U.S. Poverty Rates that are also updated
annually.
Definitions:
Household: Consists of all persons who occupy a
house or apartment whether they are related ort not.
Family: A group of two or more
persons related by birth marriage or adoption who
live together.
Income: Total annual
case receipts before taxes from all sources including,
but not limited to:
- Wages/Salaries before
deductions
- Net receipts from self
employment
- Net receipts from farm
- Self employment
- Social security payments
- Railroad retirement
- Unemployment compensation
- Strike benefits
