VISION

OPHS will substantially improve disparate health conditions * for the targeted population by:

  • Collaborating and partnering with our community members to provide culturally-sensitive, comprehensive, primary health and preventive services, which are supported by the community and staffed by high quality professionals.
  • Operating in a cost-effective and financially responsible manner.

* Infant mortality, teen pregnancy, asthma, diabetes, oral/mental health, low birth weight and high cancer rates.

SCOPE OF SERVICES

At full operation, OPHS will provide pre-natal, pediatric, adolescent, adult, dental and social work services on the platform of our company values: integrity, excellence, compassion and respect.

STATISTICS

Pontiac population: 67,166

  • With a median household income of $29,653, 51% of population is below 200% of the federal poverty level ($39,821 household income for family of four).
  • 32% of the population is uninsured
  • Compared to the county-wide rate of 6.5/1000 live births, Pontiac’s infant mortality rate is 18.7/1000 live births.
  • Pontiac ’s asthma rate is twice the state rate.
  • Pontiac ’s diabetes rate is more than three times the county rate; furthermore, 64% of the surveyed population reported that they had gone without medication because they couldn’t afford them. 31% reported that they had been hospitalized or had an emergency room visit within the last six months due to complications from diabetes.
  • 18% of the adults in the surveyed population reported that they had lost 6 or more teeth due to decay, infection or gum disease.
  • Pontiac has twice the state substance abuse rate.
  • 36% of Pontiac’s 10 th graders reported being severely depressed and/or have considered or attempted suicide.
